People leaving hospital in the city after a trauma often have new, sudden and complicated needs, and families who have had no time to prepare.

If you are a discharge planner, social worker or ward nurse, call us while the person is still on the ward. We will tell you the same day whether we can take the referral and exactly what we need from you. If you are the family, call whenever you like — you do not need to know the NDIS language, and you do not need to have anything sorted before you ring.

After a trauma admission the hardest part is usually the first fortnight at home. We front-load the support in that fortnight, then step it down as routines settle, rather than starting thin and hoping.

1   Call, any hour
Paige, our receptionist, takes your details and puts you through to the on-call team. Out of hours, someone calls you back at the time we promise.
2   A nurse assesses
A registered nurse meets the person — on the ward or at home — and writes the support plan with the family and the coordinator.
3   Support starts
A small, consistent team. The same faces, trained for the specific supports in the plan, with the nurse checking in on a schedule you agree.

Can you start before the person leaves hospital?
Yes, and it works better that way. Our nurse can meet the person and the ward team before discharge, so the first shift at home is not the first time anyone has met them.
What if the NDIS plan isn’t sorted yet?
Call us anyway and tell us where the plan is up to. We’ll be honest about what can start now and what has to wait for funding, and we’ll point you to the right person if it isn’t us.
Do you work with the support coordinator?
Always. High-intensity supports, nursing, SIL and respite come from one provider with one point of contact, which makes the coordinator’s job easier and the plan easier to manage.
