High Intensity Daily Personal Activities are NDIS supports for participants who need help with daily tasks that carry a higher clinical risk: enteral (PEG) feeding, tracheostomy care, ventilation, complex bowel care, subcutaneous injections, complex wound care, dysphagia management and seizure management. Complex Care Continuum is a registered NDIS provider in Perth (registration group 0104, High Intensity Daily Personal Activities) delivering these supports through workers trained and assessed against the NDIS High Intensity Support Skills Descriptors, with registered nurse oversight. We support participants at home, in supported accommodation and through hospital discharge across Perth’s northern suburbs.

How we keep high intensity supports safe
Every high intensity support is delivered under a written care plan prepared with the participant’s treating health professionals. Each worker is trained on that specific plan, assessed as competent by a registered nurse before working alone, and reassessed regularly. Plans are reviewed after any incident, change in health, or hospital admission. Families are told who is coming, and the team stays consistent.
Who this is for
Participants with spinal cord injury, acquired brain injury, motor neurone disease, muscular dystrophy, cerebral palsy with feeding or respiratory needs, complex epilepsy, or any condition that requires these supports as part of daily life. We also support families who have been delivering these tasks themselves and need trained workers so they can be parents, partners or children again.
Funding
High intensity supports are funded from the Core Supports budget under Assistance with Daily Life and claimed at the high intensity price limits set in the NDIS Pricing Arrangements. Supporting evidence from a treating health professional is usually required at planning. Our intake team can check a plan and tell you exactly what is fundable before you commit.
Frequently asked questions
Are your workers nurses?
High intensity supports are delivered by trained disability support workers under registered nurse oversight, which is how the NDIS intends these supports to work. Where a task requires a nurse, we provide community nursing under our Community Nursing Care registration.
Can you start quickly after a hospital discharge?
Often yes. Contact us as soon as discharge is being planned so training on the participant’s care plan can happen before they come home. Our Hospital to Home guide explains the process.
