Community Nursing for NDIS Participants in Perth

Complex Care Continuum provides NDIS community nursing across Perth, delivered by registered nurses and by support workers under registered nurse delegation and oversight. We are registered with the NDIS Quality and Safeguards Commission for Community Nursing Care, so nursing and daily support can come from one provider and one care plan rather than two organisations that never speak to each other.

What our nurses do

  • Nursing assessments and written care plans, reviewed as needs change
  • Wound care and pressure injury management
  • Medication management, administration and Webster pack oversight
  • Continence, catheter and stoma care
  • Enteral feeding (PEG, PEJ, nasogastric) setup and review
  • Tracheostomy and respiratory care, including ventilation support

Why nursing and support under one roof matters

Most participants with complex health needs end up with a nursing provider, a support work provider and a coordinator, all working from different notes. When something changes at 9pm, nobody is sure who to call. We keep the clinical plan, the roster and the escalation path in one place. The nurse who writes the plan trains the workers who deliver it, reviews it after any incident or hospital admission, and is reachable through the same 24/7 line the family uses.

Who we support

NDIS participants living with spinal cord injury, acquired brain injury, progressive neurological conditions, complex epilepsy, respiratory conditions, chronic wounds, and any disability where health needs and daily support overlap. We work alongside GPs, specialists, hospital teams and allied health rather than around them.

How community nursing is funded

Community nursing is funded from a participant’s Core Supports budget under Assistance with Daily Life, using the Community Nursing Care line items, with the level of nursing (standard, complex or specialised) matched to the task. Some plans fund nursing through Improved Health and Wellbeing instead. Our intake team can read a plan and tell you what is claimable before anything starts.

Are your nurses registered?

Yes. Nursing is delivered by nurses registered with AHPRA. Where a task can safely be delegated, a trained support worker performs it under the nurse’s plan and supervision, which is how the NDIS intends high intensity supports to work.

Can you provide nursing after a hospital discharge?

Do you visit at home?

Yes, in the participant’s own home, in supported accommodation, or in short-term accommodation across Perth’s northern suburbs.
